FIRST Student Team Members and Parents/Legal Guardians Information System and FIRST 2009-10 Consent, Release, Hold Harmless and Authorization to Reproduce Physical Likeness (“Consent and Release”) Form

This year for the 2010 FRC season, FIRST is piloting a system to collect data on each FRC student team member and their parents/legal guardians.  This system will be implemented through the newly created Student Team Members and Parents/Legal Guardians System. The overall goals of collecting this data will allow FIRST, for the first time, to:

• Create a database of FIRST student members, and their parents/legal guardians (starting with FRC teams this year)
• Connect student team members and their parents/legal guardians with FRC teams
• Streamline the annual Consent & Release Form process, and Team Rosters, to an electronic paperless method and provide a management tool for Team Leaders
• Accept submission of FIRST awards by authorized students
• Enable a reporting system on FIRST participants (gender, ethnicity, time involved with FIRST, etc.), for improved understanding and statistics
• Communicate directly with FIRST participants currently on teams, and after leaving a team – give updates, receive FIRST E-Newsletter, acknowledge and recognize participation, etc.
• Establish a reliable database that can be used for evaluation studies, and tracking participants

  2010 Consent and Release Form 

1. Student Team Members – please download this form ONLY if you are not submitting an electronic version via FIRST Student Team Member Information System;
2. Team Mentors – please download and execute this form for the 2010 season.  You will need to submit one (1) form for Kickoff attendance, and one (1) form at your initial Regional Event.

Are you attending a Michigan District Event?

Team mentors and students attending the Michigan District Events should complete a hard copy Consent and Release Form this season. This Form now covers the attendee at any FIRST event this season: Kickoff, District Event, a Regional event, the Michigan State Championship and the FIRST Championship in Atlanta. You only need to fill this form in one (1) time and bring it with you to your Kickoff. If you are not attending a Kickoff, bring it with you to your first season event. Note: If you are a student team member that has already entered your information in the FIRST Student Team Members and Parent/Legal Guardian Information System and completed an electronic Consent and Release Form, we ask that you download the form at the above link,complete, and bring it to your Michigan Kickoff event in addition to having completed the electronic form. Again, if you are not attending a Kickoff with your team, please bring to your initial District Event.
